Lakewood Rides Transportation

Lakewood Rides employee and participants.

Lakewood Rides offers door-through-door transportation for Lakewood residents 60 years of age or better and those with disabilities of any age. Call 303-987-4826 to request a ride. Strict health code guidelines have been implemented to ensure the safety of riders.

How to utilize Lakewood Rides:

1. Complete an intake form.

2. All individuals age 59 and younger must also complete a verification form.

Request a Ride

Instructions to request a ride:

All rides are by appointment only. Ride requests are filled in the order that they are received.

  • To place a ride request, call 303-987-4826. If there is no answer, please leave a message.
  • Provide the following information in your request:
    1. The day of your appointment.
    2. The address and doctor’s name and suite number, if applicable.
    3. The time you need to be there.
    4. The time you need to return.
  • After staff receive your request, they will check schedules and call you back to let you know if the service is available and when your pickup and return time will be if the time slots are available.

Step 1.Time frame to submit ride request:

Up to 2 weeks maximum for all rides. All rides are scheduled on a first-come first-served basis.

Step 2.Group grocery trips:

For the group grocery trips, we need no less than 24-hours’ notice. Please call us no later than 12:00 noon the business day before the scheduled day for your area, as trips fill quickly. (for Mondays call us on Friday before 12:00 noon)

Step 3.Cancellations:

If you need to cancel your ride for any reason, please give staff 24-hours notice (if possible) so that they can offer the ride to another person.

On-demand trips

Demand/response transportation includes independently scheduled trips for medical appointments, employment, adult day programs, social visits, and personal needs. These trips are individually scheduled and are on a first-come, first-served basis.


Dining site trips

The City of Lakewood hosts a popular and active Volunteers of America (VOA) lunch site at Clements Community Center. Lakewood Rides provides no-cost transportation to and from this location, as well as all other VOA dining sites within the Lakewood Rides destination boundaries.

Group grocery trip

Group grocery shopping trips are offered each week. Residents are picked up in their area on a designated day of the week and taken to the local King Soopers.  

GUIDELINES

To keep our trips safe and efficient and also allow drivers to assist all shoppers with their grocery needs, please follow these guidelines. 

  • Only six (6) plastic grocery store bags per shopper are allowed. Canvas bags may be used if their size is equivalent to that of the regular

    plastic bags. Larger bags become very heavy when filled and can be unsafe due to weight.

  • Be sure that all purchases fit into the six (6) bags. Example: one 24-pack of water is equal to one bag.

  • Limit your large-item purchases so that no single item exceeds 20 lbs. Example: pet food.

  • Total weight per grocery bag should not exceed 20 lbs. For reference, a gallon jug of milk weighs approximately nine (9) lbs. One two-liter bottle of soda weighs approximately five (5) lbs.

  • Please note that the driver cannot carry items such as large plants, Christmas trees and furniture.

  • Drivers are discouraged from going into anyone’s home and will place your groceries inside your door.

  • 24 hours' notice is advised to be added to the grocery pick up schedule.

Cost

There are no fares for Lakewood Rides thanks to the generous support of the Older Americans Act. Contributions are welcome but not required.

Step 1.What is an Area Agency on Aging?

The Area Agency on Aging (AAA) is a regional organization that is designated by the state to use federal, state, and local funding to meet the needs of older adults in their community. The AAA provides services to older adults and caregivers directly and through partnerships with community provider agencies. AAAs also advocate for older adults.

Step 2.Service Information

The service you are requesting is funded through the Older Americans Act (OAA) and/or Older Coloradans Act (OCA) funding. This federal and state funding helps older adults, 60+, remain in their homes and communities of choice. We are required to prioritize services for eligible individuals with greatest economic and social needs. Requests for services are processed as funds allow. We can provide you with referrals to other resources in your area, but we will not reach out to them without your permission.

Step 3.What is the purpose of the client assessment?

We ask you to provide information so that we can:
o Offer services that best meet your needs
o Prove that our taxpayer-funded programs only serve eligible individuals
o Prove that we serve older adults and caregivers most in need of services
o Understand the needs of older adults in our community
o Show the need for funding our programs
o Meet reporting requirements from our funders

We do not use income information to determine your eligibility for services. Income and other demographic information (e.g. gender, race, ethnicity) are collected for anonymous demographic reporting purposes. No personal information, such as your name or date of birth is disclosed in reporting.

Step 4.What happens with the information from my client assessment?

We enter your information into a secure state database. As you receive services, we record the services you receive in the database. This helps us prove how we spent our funding. The database is secured to the standards outlined in the Health Insurance Portability and Accountability Act (HIPAA) and Health Information Technology for Economic and Clinical Health Act (HITECH). Only authorized staff have access to the database. This means your data remains safe and confidential.

Step 5.Will you sell my information?

No. We will never sell your information.

Step 6.How do I provide feedback?

We love hearing how we can improve. Contact your service provider or your local Area Agency on Aging at 303.455.1000 or AreaAgencyonAging@drcog.org. Because we value your input, we may at times send you a survey to ask for your feedback.

Step 7.How do I file a complaint, grievance, or appeal?

You have the right to file a complaint or grievance with your service provider. If you are not satisfied with the provider’s decision, you can appeal the decision to your local Area Agency on Aging (AAA), and/or the State Unit on Aging (SUA). We accept donations and gifts to contribute towards the cost of services and to support older adult programs. Every dollar we receive goes back into the programs and services. Discrimination Complaint Procedure

Any person who believes she or he has been discriminated against based on race, color, or national origin by the City of Lakewood’s (COL) Lakewood Rides Program may file a Title VI complaint by completing and submitting the Title VI Complaint Form.

The COL’s Lakewood Rides Program investigates complaints received no more than one hundred and eighty (180) days after the alleged incident. The COL’s Lakewood Rides Program will process complaints that are complete.

Once the complaint is received, the Older Adult and Transportation Supervisor and the Family Services Manager will review the complaint to determine if our office has jurisdiction. The complainant will receive an acknowledgement of the letter informing her/him whether the complaint will be investigated by our office. The assigned staff has ten (10) days to investigate the complaint.

If more information is needed to resolve the case, the complainant will be contacted. The complainant has five (5) business days from the date of the contact to send requested information to the investigator assigned to the case. If the investigator is not contacted by the complainant or does not receive the additional information within five (5) business days, the investigator can administratively close the case. A case can also be administratively closed if the complainant no longer wishes to pursue their case.

After the investigator reviews the complaint, she/he will issue one of two letters to the complainant: a closure letter or a letter of finding (LOF). A closure letter summarizes the allegations and states that there was not a Title VI violation and that the case will be closed. An LOF summarizes the allegations and the interviews regarding the alleged incident, and explains whether any disciplinary action, additional training of the staff member or other action will occur. If the complainant wishes to appeal the decision, she/he has 15 days after the date of the closure letter or the LOF, to do so.
