Lakewood Possibilities Fund Requirements

To ensure these funds are available to those who need them the most, we need to make sure you live in Lakewood and match our income requirements. Please provide one form of documentation (for the head of household listed on the application) for both residency and income as listed. If you are unable to provide proof of residency or income, please explain your situation on the application form so that we can still consider your application.

Proof of Residency

  • Driver's license or identification card
  • Utilities bill
  • Lease agreement
  • Vehicle registration
  • Current letter from homeless shelter or support services

 

Proof of Income

  • Pay stubs for the most recent month; please indicate whether you are paid weekly, biweekly or monthly
  • Tax return (most recent year)
  • Unemployment award letter
  • Temporary Assistance for Needy Families (TANF) award letter
  • Social Security award letter (AD&D)
  • Current letter from homeless shelter or support services
  • Financial Aid award letter
