Step 1.Contact the City Clerk's Office for the Individual Background Investigation Packet

Step 2.Gather required documentation

• Current personal financial statement or balance sheet and income account statement for the preceding twelve months prior to date of application
• Three letters of character reference

Step 3.Call the Lakewood Police Fingerprinting Department to schedule an appointment

Step 4.On day of your appointment 

• Arrive at the City Clerk's Office 30 minutes prior to your appointment
• Submit application, documentation, and fees to the City Clerk's Office
• Fingerprinting fee $16.50

Fee made payable to "City of Lakewood" by check or money order.
