Change in Corporate Structure

New officers, directors, managing members and anyone with 10% or more must get fingerprinted through the Lakewood Police Department and go through a background investigation.

Step 1.Complete the State of Colorado application.

Step 2.Complete the City of Lakewood application.

 

Step 3.Submit the application, supporting documents and checks to the City Clerk's Office.

 

Step 4.Schedule a fingerprint appointment.
