How to Apply for Lakewood Possibilities Fund Assistance

You can download the English(PDF, 199KB)  or Spanish(PDF, 193KB)  2026 application and submit your forms, including proof of residency and income, by emailing them to LPF@lakewood.org. This is a secure mailbox where your information is kept safe. Dependents 26 years and older must apply individually.
