Lakewood Possibilities Fund Requirements

To ensure these funds are available to those who need them the most, we need to make sure you live in Lakewood and match our income requirements. Please provide one form of documentation for both residency and income for individuals 26 years and older. Dependents 26 years and older must apply individually. If you are unable to provide proof of residency or income, please explain your situation on the application form so that we can still consider your application.

If funds are approved, once an individual has exhausted their own funds, they may not access funds belonging to other family members. Trying to do so is a violation of our code of conduct and any future attempts will result in an inability to use our facilities and/or access LPF funds.

Proof of Residency

  • Utilities bill
  • Lease agreement
  • Vehicle registration
  • Current letter from homeless shelter or support services

 

Proof of Income

  • Pay stubs for the most recent month; please indicate whether you are paid weekly, biweekly or monthly
  • Tax return (most recent year)
  • Unemployment award letter
  • Temporary Assistance for Needy Families (TANF) award letter
  • Social Security award letter (AD&D)
  • Current letter from homeless shelter or support services
  • Financial Aid award letter

Funds awarded in 2025 must be used by December 31, 2025.

We are now accepting applications for 2026. Funds will become available January 5, 2026. Funds awarded for 2026 will expire December 1, 2026.
