Due to the nature of this type of report, no further investigation is anticipated. This report is for information or insurance reporting purposes only. Information submitted using this website is secure.

AFFIDAVIT

Reporting of any crime that is false or malicious is punishable by law. All violators will be prosecuted for filing a false police report to authorities. Police report information is public record.
 
If you have provided an e-mail address, you will receive a confirmation email indicating receipt of this report, which will contain a case report number. Remember, it is highly unlikely that you will receive any further investigative contact from the police department due to the nature of this report. An official police report will be generated.
To obtain the official police report filed with the Lakewood Police Department Records Section, you can submit submit the Police Records Request Form in person or by mail and pay the required fee.
