Requirements for Passports

Before arriving for your appointment you must do the following items:

Complete all necessary U.S. Department of State forms

Please remember that your forms need to be printed out single sided.  If handwritten, please use only black ink.

Gather your proof of citizenship

State certified birth certificate, previous passport, or a naturalization certificate.
You must submit one copy of your proof of citizenship along with the original. Copies must be supplied of the front and back of the document and single-sided.

  • For minors 15 and under, child and both parents listed on the birth certificate must be present. If the minor has been issued a passport before, they must submit their birth certificate and previous passport. Passports issued to minors cannot be renewed.
  • For minors 16-17, minor and one parent listed on the birth certificate must be present.

Gather your proof of identity

Valid driver's license (not a temporary or learner's permit), naturalization certificate, previous U.S. passport, government-issued I.D. with photo and signature, Military I.D. with photo and signature. Copies must be supplied of the front and back of the document and single-sided.

Obtain and bring one 2 inch by 2 inch color photograph with white background

Passport photos are not available at our location.

Pay a fee to the U.S. Department of State

Fees for passport books, passport cards, expediting fees and over night MUST be paid to the U.S. Department of State by check or money order only.

Pay a fee to the city of Lakewood

  • Fees of $35 for execution can be paid to the city of Lakewood in the form of cash, check, money order, VISA, MasterCard, or Discover.
  • There will be a charge of 25 cents for each copy made, including photo ID's.
  • Passport Photos are NOT available at our location.

The city of Lakewood is a designated Passport Acceptance Facility under the auspices of the U.S. Department of State.
