A RESOLUTION
FOR RENEWAL OF CONTRACT FOR THE 2008 CONCRETE REPAIR PROGRAM.
WHEREAS, by Invitation of Bid No. 2580, the City of Lakewood advertised for bids for the 2006 Concrete Repair Program and bids were opened on January 8, 2006.
WHEREAS, the City entered into a contract with the low bidder, Fasick Concrete, Inc., which included a provision for 2 contract renewals at the option of the City of Lakewood.
WHEREAS, Fasick Concrete, Inc. performed the 2006 and 2007 Concrete Repair Program in a quality, efficient manner.
WHEREAS, Fasick Concrete, Inc. has requested an increase in contract unit prices of 4.5% for renewal of the contract in 2008 which the City of Lakewood wishes to accept for performance of the 2008 Concrete Repair Program.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Lakewood, Colorado, that:
SECTION 1. The renewal prices submitted by Fasick Concrete, Inc. for the 2008 Concrete Repair Program as described in Invitation to Bid No. 2580 are hereby accepted according to specifications as delineated in the original Invitation to Bid.
SECTION 2. The City Manager is hereby authorized to execute, and the City
Clerk to attest, a contract with the said contractor in form approved by the
City Attorney in the total amount of One Million One Hundred One Thousand Four
Hundred One and 79/100 Dollars ($1,101,401.79), The City Manager is hereby further
authorized to execute change orders to said contract, in form approved by the
City Attorney, which shall not exceed, in the aggregate, an additional ten percent
(10%) of the contract or One Hundred Ten Thousand One Hundred Forty and 18/100
Dollars ($110,140.18), for a total contract amount not to exceed One Million
Two Hundred Eleven Thousand Five Hundred Forty One and 97/100 Dollars ($1,211,541.97).
SECTION 3. The City Treasurer is hereby authorized to make payment to Fasick
Concrete, Inc.
INTRODUCED, READ AND ADOPTED by a vote of 11 For and 0 Against at a regular meeting of the City Council on January 28, 2008, at 7 o'clock p.m. at the Lakewood Civic Center, 480 South Allison Parkway, Lakewood, Colorado.
