The following forms are for your use in submitting a "Typical" Traffic Control Plan. Please use these forms only if you need a "Typical" TCP. If you are unsure as to whether or not you can use one of these forms, please contact us at the number listed below.

These forms are in .pdf format and may be filled out on-line, but cannot be submitted on-line (you will need to download Free Adobe Acrobat Reader (PDF) to view these forms). Please type in the required information, print the form and submit it with your Permit Application to the Permit Counter at the City of Lakewood.
