Welcome to the Camp Paha Registration Paperwork page. Here you will find all forms you will need to register your camper for Camp Paha Summer 2009.

If your camper has been guaranteed a spot at camp, you will need to fill out Application Materials here.

Camp Paha Summer 2009 Registration Day

When: Saturday, April 4th from 9am until 12pm

Where: Whitlock Recreation Center, 1555 Dover Street, Lakewood, CO 80215

What: To expedite the registration process, please complete the following forms before registration day:

1. Parent Information Sheet
2. Camper Registration Form
3. Payment Plan Information and Agreement
4. $25 non-refundable registration fee (make checks payable to City of Lakewood)
