Camp Paha Summer 2009 Application Materials

Please note that these materials are intended ONLY for campers who have been guaranteed a spot at camp. If your camper is on a waitlist, DO NOT fill out these materials.

If you need to Register your camper for camp, please visit the Camp Registration Page and follow the directions.

Please note that all forms are in .pdf format. To view these forms, you will need the free Adobe Acrobat Reader, available here.

If you successfully registered your camper at the Camp Day registration (Saturday, April 4th 2009), you now need to complete the following forms for your camper (all forms in .pdf format):

1. Camper Application Cover Letter (63 Kb)
2. Camper Information sheet for Counselors (76 Kb)
3. Camper Physical and Fieldtrip Form (49 Kb)
4. Camp Policies and Procedures (85 Kb)
5. Acknowledgement of Camp Policy and Procedures Form (36 Kb)
6. Medical Exam and Permission to Medicate Forms (43Kb)

Please note Application Materials are due no later than Friday, May 29th, 2009. Please submit your forms in a timely manner. All forms can be mailed to:
