INTEREST CARD RULES AND DIRECTIONS

 

  1. The cards are being issued for Lakewood Housing Authority’s combined waiting list of Public Housing and Section 8 Housing Choice Vouchers.
  2. You must be 18 years of age or older. 
  3. Only one card per family will be accepted.
  4. ALL QUESTIONS ON CARD MUST BE FILLED OUT AND CARD MUST BE SIGNED AND DATED.  ALL INCOMPLETE CARDS WILL BE DESTROYED.
  5. Cards must be mailed in and postmarked by the deadline date of February 6, 2008.  Hand delivered cards will not be accepted.
  6. INCOMPLETE CARDS AND CARDS POSTMARKED AFTER THE DEADLINE DATE WILL BE DESTROYED AND WILL NOT BE ADDED TO THE WAITING LIST.
  7. You will be contacted at a later date by mail with instructions regarding how to proceed with the application process.
  8. If your mailing address changes after you return your card, you must submit the address change in writing and the change must include your Social Security number and signature.  The written change must be sent to Lakewood Housing Authority, Attention: Waiting List Technician, 480 South Allison Parkway, Lakewood, CO 80226.

NOTICE OF AVAILABILITY OF REASONABLE ACCOMMODATION


It is the Lakewood housing Authority’s (LHA) policy to provide “reasonable accommodation” in housing for applicants, residents, and clients with disabilities who are otherwise qualified for LHA’s services.  This policy is in furtherance of LHA’s goal of providing affordable housing to low-income persons regardless of disability and in compliance with applicable federal, state, and local law.

A person with a disability is one who (1) has a physical or mental impairment that substantially limits one or more major life activities, such as caring for one’s self, doing manual tasks, walking, seeing, hearing, breathing, learning and working; (2) has a record of such an impairment; or (3) is regarded by others as having such an impairment.

A “reasonable accommodation” is a modification or change the LHA can make to its procedures and rules, or to the person’s apartment or to a public/common area where such is necessary to provide a person with a disability an equal opportunity to participate in and benefit from LHA housing, aid, benefits, programs or services.

A reasonable accommodation may also include provision of an appropriate auxiliary aid where such assistance is necessary to enable effective communication between the applicant or resident/client and LHA.

If you or a member of your household have a disability and think you need a reasonable accommodation, you may request it in writing at any time during the application process or after admission.  This is up to you.  If you would prefer not to discuss your situation with LHA, and not request a reasonable accommodation, that is your right.
